No medications may be administered at school unless the nurse has written permission from the parent. A medication administration form must be filled out and signed by the parent. If medications are to be administered for more than two weeks, a doctor’s signature must be obtained. All medications must be in
the original container. Medications brought to school in a “baggie” or any other unauthorized container will not be given.
Students are not allowed to carry any medications at school unless a form is on file with the school
nurse. Students are allowed to carry emergency medication such as glucose tablets for diabetes, glucose machines, allergy pens, and inhalers as long as the proper forms are filled out and on file in the office. Students are not allowed to transport medications to school. Parents must arrange to bring the medication to school.
Emergency medication for potentially life threatening situations also require a signed authorization from the parent or guardian, and a written order form the health care provider that must include:
Students who are ill during the school day must see the nurse in order to check out. We do not have a “check out list”. A parent must be contacted for check out. Students are not allowed to use cell phones to call home. The call must be placed from the clinic.
Any student who is diagnosed with a contagious illness must bring the school nurse a note from the
doctor clearing that student to return to school. Examples of contagious illnesses are: mono, strep throat, flu, conjunctivitis (pink eye), staph infections.

Any necessary aftercare
